I'm almost certain Utada has had a bit of work done on her eyes, and given her release schedule, she'd definitely have time for surgery. It's possible that her pre-fame pictures were particularly bad/low-quality photos, as her double eyelids aren't as defined as Ayumi's, but then again, surgery for artists is nothing new. In Utada's case, it's not as if consumers purchase her music for her looks, so it's not a big deal.
A bigger deal is whether Ayumi has had work done, since one of her strengths is her looks. Given that she works 24/7, it's not unusual for her skin to age faster than normal. If it can happen to newly-patented doctors in their late twenties, it can happen to Ayumi. I figure if she has time to take care of her 8978412 dogs and hang out with her former boyfriends, she has time for a little surgery.
Ayumi now without makeup resembles an older, thinner version of herself ten years ago. That itself limits what kind of surgery she could have had, but it doesn't rule out surgery either. The woman still looks good, which I think everyone agrees on.
(On a side note, in my previous post, the artist I mentioned with possibly a "teensy bit of work done" is Ayumi, not Utada. Just to clear that up.
