I think a lot depends on hairstyle/makeup. And she did gain weight; I wouldn't call her fat by any stretch of imagination (in all honesty she was SUPER skinny in 2003, and it didn't actually look that healthy). Although I think some of it is due to her choice in wardrobe, which some would actually make her look skinnier and some would make her look fuller (like the puffed sleeves in Secret era).
I think she's wearing heavier makeup nowadays, and I think some part is due to the fact that she is older and the heavier makeups might make her look more flattering and look younger.
Although on the Christmas live I did see dark shadows underneath the eyes that shown through makeup a little.
