I was thinking about it when reading Utada's "Forevermore" translation. It could be interpreted as romantic love, but I guess it's written for her child ("others come and go but you're in my soul forevermore") and chorus it's just like "aishiteru, aishiteru". I think it's a word you use when you want to express a extremely deep bond.
But I don't know japanese and its nuances.
