Does anyone know what the bit in Japanese at the top of the form says? I can figure out that the drop-down menus are for your first, second, and third song choices, but I don't know about the rest.

2nd, asks for your member number
3rd, name
4th, age
5th, first option: male. second option: female.
then you pick the top 3 songs you wanna hear/see

but after considering for a while (I listened my choices again - POWDER SNOW. For My Dear..., Wishing, fairyland, GUILTY, Far away), I decided to vote:
