It's real. I've seen her live many times. She does often use a backing track; which I actually prefer to having backup singers. That does tend to give you more of a sound like on the album, but she is still singing live over it and it's only for certain sections of songs.

It depends. I went to a few shows last year and her performance really depends on how she feels that day. She has no problem with dance songs, but when it comes to ballad, I have to say that Love Story was a disaster in live.. at least for the one that I went to.

I've been to her concerts for two times, and some dance or upbeat song does has A LOT of backup voice that almost cover her own voice, which kinda make her sounds like what Perfume (electronic music group trio) does in their concert. I think she has a lot of "resting" moment during upbeat songs (I could be wrong though)

I always thought Namie has one of the most excellent concert in J-pop music. I understand a lot of people might think she looks lazy when she is dancing, but at least her stage design and the visual thing is new in every tour.

DOn't get me wrong, she is my Nr. 1 artist and her shows are awesome as fuck - but I still wish for something new.

Anyway, after redownloading Best Fiction Tour I watched it for the first time in a while and I forgot just how hard it was to watch some of the performances. Was she sick? Or just overdoing it? Or both? She seemed really exhausted throughout half the show. Which I totally don't blame her for, of course, seeing how long the show was and how many dance numbers there were back-to-back. As a fan though, I actually worried a bit xD I'm happy she doesn't try so hard (as in over-doing it) in her more recent tours. She seems much more relaxed nowadays than she did then.

Does anyone ever think that her concert's setlist is a bit... random? For example, a random ballad song sandwiched in a few dance track or maybe a cute song appeared right after a ballad etc

^I don't think so. It's the same as her albums and probably serves as a point for a rest for her during the concert.

it's not. she performed it to death and the album is good even without it + GT was the song with more attention in Japan.
At least this way there is more to look forward to when we get a b-side collection someday (not that own the +DVD Version of GIRL TALK)

Regarding the news about ayumi hearing and knees problem, I'm actually quite amazed that Namie did not suffer the same problem as her because Namie did not even wears ear-pieces during her concerts until her 2012 dome tour! And dancing for hours in high heels did not seems to give her any problems with her legs... I wonder how she does that!
