so it mean that she used her mother's surname?

If they're both named "Hamasaki" I think that would be a big yes then, right? ^_^

But...she doesn't talk about him, she doesn't know his name, he abandoned her mother and her when she was a child...I don't think she would go and dedicate a song/mini-album to a man who I seriously doubt she holds any kindness towards.

death (and time) can have a huge impact on people, though, and make them reconsider things. She *did* go back to where she grew up and visited her old school and a beach where she said her father took her to play (according to the NTV special, anyways...) maybe she wishes that she had a father figure in her life. It's been a few years since that interview gohan posted...
