Koda, I believe, did that as a part of her "edgy" image... Like, the bold girl... She was smiling and all...
Madonna is always flipping her finger and calling her audience "mother*********s" on concerts... In the end of the day, the "rules" of behavior on concerts are different from a TV show or PVs... the artist has more freedom cuz they are being watched by fans, not by the general public...
