I always thought she just takes a Japanese to English dictionary and chooses words that match meanings that she's trying to get across..

^Yes, "pain" is "itami". You can also use "kurushimi", but that is quite often translated as "suffering".
And if you use "ka", you have to use it with both words, as far as I know. So it would be "KIBOU ka ITAMI ka". I think "ka ... ka" translates to "either... or", but I'm not sure.
