I don't think it's simple for a doctor to determine whether a aesthetic plastic surgery was done or not, since facial features change a lot, and I'm sure we'd be getting interviews with doctors through tabloids explaining evidences of plastic surgery on these celebrities, something we don't really get.
 unless the changes were extremely obvious and we can compare two recent pictures, one after and other before surgery, that clearly proves that.
