your suggestion:

Are certainly there, but..."

tweaked:

"painful, painful
nights, when my chest feels so tight,
are certainly there, but..."

again, I took some liberties to make the english not sound so awkward whilst keeping the order of information in tact (as much as possible)

I'm quite happy with it...any thoughts?
