I think you can submit once only. But there's 4 options for you to choose different dates. But if you do choose different dates, there's a chance for you to win both options. And you'll have to pay for both of the dates won as they won't give you options to pay for only 1 of the dates. So choose wisely. It's a ballot so it doesn't mean that your ticket is confirmed unless you're picked.

OMG Really??? I only wanted to go to the 30th but I thought of putting the May 31st too just incase. Does it mean if i got picked in both dates, I have to pay for all?

No, I think Ayu designed it like that and said there was a 'secret' to the title which is why people came up with that to begin with.
