The cover could simply refer to how she feels, how she has to be careful at what she will say, because the past year, people keep jumping on her, like how she had her staff take over her Twitter, because her japanese fans kept jumping on that she was cheating on her husband over some remarks she made with male friends. It could be a reference to today's social standings and how people react over the littlest thing now a days, that you feel the real problems can never come out. Or it was just a simple idea from her photographer and like said by another user in a few posts ago, it could of just been a 5 min photo shoot
